1950-08-04 Mount Royal, Quebec:
On a sightseeing flight from Cartierville Airport engine stopped. A forced landing (wheels retracted) in a field west of the town of Mount Royal resulted in serious damage to the aircraft when it nosed over, but no injuries to the four occupants - pilot John Hardcastle Waldie (C-5091, 22 years old) and three passengers. Investigations disclosed that the cause of accident was attributable to lack of fuel. Waldie's Commercial Pilot's Certificate was suspended for 30 days: from September 8th to October 7th, 1950.

Shortly after taking off, engineer Narcellin Boldue informed Curtiss-Reid president W. R. J. Oliver that he believed the aircraft did not have enough fuel for a flight of any duration. Oliver called Cartierville tower, asking them to contact Waldie to make him immediately return to airport. Unfortunately, Waldie did not fully hear the message due to static noise. Still, Waldie was in the process of returning to base when engine stopped.

1959-08-06 Letter from DoT, Regional Superintendent, Air Regulations, Montreal to Curtiss-Reid Flying Service Ltd. informing that last Certificate of Airworthiness # 2516 lapsed on November 25th, 1950.
"In view of the considerable length of time that this aircraft has been inactive, you are requested to immediately advise this office as to the present status of ownership, airworthiness and location of this aircraft".
1950-08-19 Letter from Curtiss-Reid Flying Service Ltd., W. R. J. Oliver, President, to DoT informing:
"The only remaining part of this aircraft which could be used is the hull. We had an idea of rebuilding this machine and assume that if this was ever done, new certificates could be issued. In the meantime the aircraft is out of service and more than likely will remain so".
