1951-12-19 Patricia Bay Airport, BC:
Aircraft failed to take-off from RWY 02. Minor damage to undercarriage, a broken clevis pin. Aircraft skidded on keel 173 yards along the overshoot area. Pilot: W. B. Sylvester (Senior Commercial SC 128). Passenger Mr. S. E. Scurrah. No injuries.

"The aircraft was taking off and did not seem to have sufficient power to keep it in the air and when over the end of the runway an emergency landing was made. When it was apparent that an emergency landing would be necessary the crash alarm wa sounded and shops called by phone to give location of aircraft". (Report Air Traffic Control Officer).

An Air Engineer report dated 1952-01-02:
"A thorough inspection of the engine was carried out after the accident but with no apparent results at to the reason for the failure of the engine at take-off; but, upon the removal and inspection of the fuel tank and lines it is believed that a restriction of fuel had taken place in the base of the tank at the main fuel outlet. There had been an accumulation of water in the sump and due to the low outside air temperature the outlet screen in the tank sump had become restricted with ice and would not permit a sufficient flow of fuel at take off power".
