Smith, Northwest Territories; 60°01'N, 111°58'W: Forced landing in the
bush due to engine failure, caused by mechanical failure of mixture
control. At about 12:27 MST aircraft crashed three miles south of Fort Smith Airport, Northwest Territories, on final approach to runway 28. Aircraft came in from Fort McMurray, Alberta. Radio technician John Karl Dietrich Lengwenus (27), single passenger aboard, was killed and commercial pilot Lloyd Ellwood Smith (37) of Calgary was seriously injured. Smith was taken to University Hospital Edmonton. Pilot reported that he was forced to land when engine stopped, supposedly due to fuel starvation. Aircraft was owned and operated by Chinook Flying Service of Calgary. Aircraft was on route to DEW-line from Calgary via Edmonton and McMurray. Both occupants were employees of Chinook Air Service. Accident investigations, however, disclosed that there still was sufficient fuel in the fuel system, and that the cause of engine stop was a mechanical failure in the mixture controls: ...it was found that the carburetor mixture control arm was in the full lean position at carburetor. The reason for the mixture control being found in the full lean position was due to the flexible casing pulling out of the retaining fitting at the carburetor and allowing the mixture control arm to assume to the lean mixture position. |
