1948-06-23 La Tuque, Province du Québec; After take off undercarriage was selected up, but before it was raised hydraulic pressure fell to zero.  Pilot Jean Rivard forced to land on hull.  Two passengers. Pilot Jean Rivard's Report (1948-06-24): 

On Wed. June the 23rd, I took-off with the above mentioned aircraft, which to the best of my knowledge, was serviceable in all respects.

My load consisted of: two passengers, 120 pounds of luggage, 15 gallons of gas and eleven quarts of oil thus giving med a total of 727 pounds including my own weight.

After a normal take-off, I selected the wheels up and starting pumping the hydraulic pump. After three or four strokes, which was just enough to unlock the wheels, I heard something fall into the hull and the pressure gave way completely. I continued my climb until I reached a safe height and then tried to locate the trouble. I opened up the anchor compartment and discovered that one of the relief or check valves had come out.

As the wheels were unlocked but in the down position I had no alternative but to land it on the hull. I landed alright but after the aircraft slowed down the right wing dropped and the float caught on the ground and broke off the strut where it is attached to the wing. The right flap was damaged slightly.

After the aircraft was lifted up and the wheels were locked down, I taxied it in to the tie-down position and we inspected it thoroughly. No other damage than the above mentioned was found.

1949-08-25 While ready for take-off, one blade of the propeller broke away from its main hub assembly, causing extensive damage to the Franklin engine main crankshaft and crankcase assembly, also a limited damage to the upper part of the fuselage stressed skin.

1950-12-23 Bostonnais River Valley, 5 miles NE of La Tuque, Quebec:
After take-off from La Tuque (on skis?), heading for Lake Bouchette, at an altitude of six-seven hundred feet, engine started to loose power. A forced landing was made in a short field. Aircraft hit trees and was severely damaged. Pilot: J. Jules Ringuette (C-1402). Passengers Benoit Gagné, Paul Drolet and Henri Bernier, all of Lake Bouchette. Gagné fractured his right foot. Aircraft was transported by truck to Lac A La Tortue for inspection.

Engine 6A8-215-B9F, S/N: 24036, 1221:10 TTSN, 511:10 hrs since major overhaul, was examined. No defects were found.

It was assumed that fuel icing or frozen fuel lines had restricted fuel flow to engine, causing the loss of power.

Damage to aircraft:
a) Leading edge of both wings dented in quite badly
b) One rear spar bent
c) One wing strut bent
d) The undercarriage position lug broken
e) Tail ski fork broken off
f) Tip of one propeller blade chipped

In addition to the above damage, the tail end of the fuselage was severely bent and torn by the truckers who delivered the aircraft back to base...

1952-07-02 Near La Tuque, Quebec:
Aircraft took off from Lac á Beauce, bound for Lac Long, Quebec, pilot L. F. T. Head and two passengers. After flying some 10 miles, the aircraft had to ditch due to engine failure. Pilot and passengers escaped with minor injuries. The aircraft, however, was written off. [Canadian CAR Files # 5008-F19-4, 5002-330].
